For the purposes of reimbursement, reporting and research, in accordance with established policies and procedures, The Coder II, Health Information Management is responsible for coding all non-Medicare inpatient and non-Medicare outpatient surgery patient type medical records according to diagnoses, operations and procedures using appropriate classification systems. Abstracts designated demographic and clinical data for use by researchers, and for reporting to external agencies for profiling and benchmarking. Provides codes to the Accounts Receivable! system to allow for the initiation of billings.
E! ssential Job Outcome & Functions:
Reviews the medical record of non-Medicare inpatient and non-Medicare outpatient surgery patient types to determine the principal diagnosis and procedure, and all appropriate secondary diagnoses and procedures.
Using the ICD-9 (International Classification of Diseases) and CPT (Current Procedural Terminology) classification systems, assigns codes to all diagnoses and procedures following applicable coding principals and department guidelines; enters into the computer coding system for transmittal to billing systems.
Following state and federal guidelines, determines those conditions not present on admission and flags accordingly for POA (Present On Admission).
Abstracts all required clinical and demographic data for internal and external benchmarking for OSHPD (Office of Statewide Health Planning and Development), Quailty Management, and Medical Staff; enters information into the computer abstracting system.
Maintain! s current knowledge of the most recent ICD-9-CM Official Guidelines for Coding and Reporting, the annual changes to the ICD-9 diagnosis and procedure codes, and the annual changes to the CPT procedure codes.
Minimum Requirements/Education/Licensure:
1+ yearsof ICD-9/CPT coding experience in an acute care hospital.
CCS/CCS-P/CPC/CPC-H/RHIT/RHIA is preferred.
Equivalent in work experience will be considered.
